1) Please complete and download new patient forms and also bring your ID card and insurance card to your initial visit.
2) We will make our best effort to check your eligibility for your insurance. We will be happy to discuss your insurance coverage with you, but you are ultimately responsible for any treatment rendered that is not covered by your dental insurance.
3) During the initial visit, Dr. Lilly will perform a comprehensive exam, dental cleaning, age appropriate x-rays, and fluoride treatment. If you do not want a specific treatment to be done, please let us know ahead of time.
4) After the initial appointment, the Dr. Lilly will discuss with the parent/guardian the clinical findings and any further recommendations. Any follow up treatment that is necessary will be discussed in detail with the parent and a treatment plan will be created.
